Undergraduate Form Signature Requirements

For Falk College Students

Add/Drop

Advisors should check that the student has at least 12 credits. The student is also responsible to make sure he/she is not below 12 credits. If “TCR” is on top of the form no advisor signature needed. If “Administrative Drop” is on top of form an instructor signature is needed but no advisor signature is needed.

Signatures Required

  2. Instructor (authorized department representative may sign for instructor)
  3. Academic Advisor or Undergraduate Director/Department Chair
  4. Student Services – 300 MacNaughton Hall (get add/drop stamp)
  5. Student submits to Registrar’s Office – 106 Steele Hall

Declaration of Major

For switching majors within Falk College or adding another major within Falk College.

Signatures Required

  1. Student
  2. Authorized Department Signature
  3. College Recorder or Assistant Dean of Student Services

Declaration of Minor

For declaring a minor or requesting an additional minor.

Signatures Required

  1. Student
  2. Academic Advisor
  3. Minor Coordinator
  4. College Recorder or Assistant Dean of Student Services

Declaration of Removal of Minor

For removal of a current minor. Before certification time, all minor/second majors not complete must be deleted.

Grade Change

Only needed for students who are NA, NR, RM or WD or who have graduated. All other grade changes are done online. The Home College’s Dean of Student Services signs off on this form, if you belong to a particular college. For example: if an Arts & Science student takes a Falk College class, that student needs to get the Dean of
Students Services in Arts & Sciences’s signature.

Grade Option – Pass/Fail

For pass/fail electives courses only.

Signatures Required

  1. Student
  2. College Recorder at 300 MacNaughton Hall (get pass/fail stamp)
  3. Student submits to Registrar’s Office at 106 Steele Hall

Grade Option – Audit

For registering a course as an audit or for switching a course from audit to credit.

Signatures Required

  2. Department Approval (where course is located)
  3. Student Services – 300 MacNaughton Hall
  4. Student submits to Registrar’s Office at 106 Steele Hall

Incomplete Grade

An incomplete grade will calculate as an ‘F’ until an actual grade is submitted.

Independent Study Proposals & Internship Proposal Agreements

Student and Faculty Sponsor must fill out the form completely and the Assistant Dean of Student Services is the sole College Signer for all proposals.

Intra-University Transfers (IUT) – Transferring into Falk College

Student should meet with the department for information. Forms are submitted to Office of Student Services for final signature.

Intra-University Transfers (IUT) – Transferring out of Falk College

Signatures Required

  1. Student
  2. Authorized Department Signature (New College)
  3. New “Home” College

Leave of Absence/Readmit

For leaving for a semester or withdrawing from the University, or for being readmitted to the University. Students should contact 300 MacNaughton Hall 315-443-3144 first. Suspended students should discuss plans with the Department Chair and then send a readmit letter to the Assistant Dean of Student Services. Medical leave students must also contact the Office of Student Assistance. Judicial students must be cleared through the Office of Student Rights and Responsibilities.

Petition to Faculty – Substitute/Waive Courses

Student must provide explanations and course descriptions for substitutions. The final destination of this form is the respective college recorder who enters data into Degree Works.

Petition to Faculty – Withdraw from a class

Advisors should check to make sure the student has at least “12 credits”. Student is responsible to make sure he/she is not below “12 credits”.

Petition to Faculty – Transfer Credit

For transferring credits to Syracuse University including credits for summer courses and non-SU Study Abroad courses. Student must attach a course description and submit it before the course is taken.
